Accessing Supports Auscare House is committed to ensuring clients access appropriate services that meet their needs, goals and preferences. Management will communicate with and provide a person-centred approach to support the needs of clients wishing to access services or supports including by providing support to access information related to supports in the language, mode of communication and terms that the resident is most likely to understand.
Reasonable adjustments to the support delivery environment will be made and monitored by Auscare House to ensure it is fit for purpose and each clients' health, privacy, dignity, quality of life and independence is supported.
Support Planning Auscare House's support planning process places the client and other members of support at the centre of the client's plan and designed to assist clients' to build capacity to have as much control as possible over the planning, implementation and review of their plan and the supports and services provided by Auscare House.
Auscare House will ensure each support plan is reviewed annually or earlier in collaboration with each client, according to their changing needs or circumstances. We ensure the support and services intervene in the life of the client in the least intrusive way possible with the smallest possible infringement on the clients' rights.
Where appriopriate, and with the consent of the resident, information on the support plan is communicated to family members, carers, other providers and relevant government agencies.
Service Agreements between Auscare House and Client When Auscare House makes an offer to provide services and support to the resident, Auscare House will deliver the resident with a completed Service Agreement. The agreement will be based on the proposed expectations, services and supports to be delivered and conditions attaching the delivery of those services and supports, which all parties involved discussed and collaborated on at prior meetings.
Auscare House will use its best endeavours to ensure all service agreements are documented in writing. In the unlikely event that a written service agreement is unable to be prepared, a record is made of the circumstances and reasons why a written agreement was not created.
When a client is transitioning from another service provider, the support planning process will also involve the development of a planned transition plan in relation to the transition of supports and services to Auscare House from the previous service provider.
Auscare House values diversity and by extension workers and clients from diverse backgrounds, religions, ethnicity, cultures, abilities, language, personal characteristics, backgrounds and situations. We have significant experience communicating effectively with people from diverse backgrounds.
We have 20 support workers who work with our clients on a daily basis. They will assist the client on outings, taking them to their medical appointments, help with self-care and everyday tasks.
